Standards for voluntary termination of a license are as follows:

1. Providers that voluntarily close or surrender their Adult Foster Care License should notify the County Social Service Agency or the agency that completed the licensing study, residents, and the resident's legal representative, if any, in writing at least thirty (30) days prior to the termination date.
2. The County Social Service Agency that completed the licensing study shall immediately notify the Aging Services Division of the providers’ intent to voluntarily terminate their license.
